This adapter has a female hotshoe that will accept a hotshoe-based flash and short cord with a male PC connector.  This setup provides a very convenient way to attach a common flash cord with a female PC end for flash units that do not have a built-in sync connector.

This adapter will provide a reliable sync connection for flash units such as the Canon 580EX, 550EX, 430EX, 540EZ, 430EZ, 420EZ, Nikon SB-600, and many other flash units that do not have a sync connector on the flash.

One common application is to connect these flash units to an optical slave such as this product from FlashZebra.com.

Another common application would be to connect these flash units to the 4 channel V1, V2, or V2s style of inexpensive radio slaves sold by Gadget Infinity, and other photographic merchants.

The bottom of the connector provides both male shoe rails and a 1⁄4 inch - 20 tripod screw.  This affords many mounting opportunities including to standard tripods, to other flash shoes, and directly to the very popular umbrella adapters.
